Please note: Physician Verification Forms submitted prior to the start of the new plan on June 1st will be accepted and credited toward the 2013-2014 plan year. Health assessments, however, may not be completed until the start of the new plan year and may only be submitted online. Detailed instructions for online access to the Good Life Health Assessment will be made available after June 1st, though a sample has been posted for review.
