Checklist to Registration

What You Need to Do:

• Apply to Daemen College
• Be accepted.
• Have paid your tuition deposit.
• Be Health Compliant.
  • Review page one of the Student Health Form so you are aware of what immunizations you MUST (in accordance with NYS law) document before being allowed to register for classes. Bring the health form to your health care provider. Make sure all required immunizations are documented (page 3).
  • Read the information on page four of the Student Health Form and make an informed decision regarding the meningitis vaccine. On page three, either enter a date of having had the vaccine or, after full consideration, sign and date indicating that you are aware of your risks, the availability of a vaccine and your intention to decline the vaccine at this time.
  • Return the Student Health Form (and any official documentation of required immunizations) ASAP.
  • All students who live on campus, all international students, all student athletes (club and intercollegiate), and those students enrolled in a health-related field of study who will be participating in clinical experiences are required to be covered by health insurance. If you are not already covered by your parents' insurance or a plan of your own, Daemen offers a plan. ALL students are eligible to emily in the plan. Coverage will begin on January 27, 2010, and will terminate on August 20, 2010. The cost is $687. Information about the plan can be found at www.gallagherkoster.com. • Have official transcripts from all institutions attended in your admissions file.
  • Contact any colleges from which you have received any college credit, to request that your official college transcripts be sent to Daemen College.
  • High school transcripts are also required unless you have received an associate’s degree from an accredited college.
  • Once all transcripts have been received an evaluation of your college credits will be processed.
• Make an appointment to register for classes.
  • Your academic advisor will be a faculty member in the department in which you plan to major.  We feel that the earlier you register for classes the greater chance you have of obtaining the classes you want and need. Please contact Joe Pagano (716-839-8232 or jpagano1@daemen.edu).
