How to Apply
In order to determine your eligibility for the Arthur O. Eve H.E.O.P program at Daemen College, please submit the following items:
1. Documentation of ALL household income for 2012:
-Federal IRS Form 1040 (Only Pages 1 and 2) with all signatures
-W-2 Form(s)
-Social Security Documentation
-Social Service Documentation
-Unemployment Compensation
-Disability Payments
-Child Support, Alimony, etc
-Other income
2. A Copy of your Completed 2013-14 Free Application for Federal Student Aid
(FAFSA) www.fafsa.ed.gov Please add Daemen School Code: 002808
3. Two Letters of Recommendation (Teachers, Guidance Counselors, Employers, etc.)
4. A Brief Autobiography, Explaining, "Why a College Education is Important to Me"
5. 
HEOP Data Form
Students applying for HEOP students must not be apply "test optional" and should include their SAT scores with all of the above information.
