How to Apply

    In order to determine your eligibility for the Arthur O. Eve H.E.O.P program at Daemen College, please submit the following items:

    1. Documentation of ALL household income for 2011:

                        -Federal IRS Form 1040 (Only Pages 1 and 2) with all signatures

                        -Social Security Documentation

                        -Social Service Documentation

                        -Unemployment Compensation

                        -Disability Payments

                        -Child Support, Alimony, etc

                        -Other income

    2. A Copy of your Completed 2012-13 Free Application for Federal Student Aid 
         (FAFSA) www.fafsa.ed.gov  Please add Daemen School Code: 002808

    3. Two Letters of Recommendation (Teachers, Guidance Counselors, Employers, etc.)

    4. A Brief Autobiography, Explaining, "Why a College Education is Important to Me"

    5. HEOP Data Form 3411 HEOP data cd 2011.pdf
