Transfer Registration Process

 

ARE YOU REGISTERED FOR YOUR FALL CLASSES?

Before you can be registered for classes you must:

1. Be accepted to Daemen (this part should already be done – congratulations).

2. Be Health Compliant

•  Complete page two of the Health Form

•  If you are going to be living on campus or if you are an international student (regardless of where you will be living while attending Daemen), you must be covered by health insurance. Complete the Health Insurance Waiver Form.  If you indicate that you are not covered by insurance, information about and an application for the DC Student Accident and Illness Insurance Program will be sent to you.

•  Read the letter about meningitis, as well as the information on page four of the health form.  Complete the meningitis section on page three of the health form by either indicating the date you received the vaccine or by signing and dating that you are declining the vaccine. You may also indicate that you have not yet had the vaccine but plan to have it within the next 30 days by writing that on the form. Once you have had the vaccine bring the documentation to the Health Services office.

•  Return these (and any official documentation of required immunizations) ASAP.  If you do not have official documentation of immunizations, complete the Health Record Release Form and forward it to the school from which you are transferring.  Include the student copy with the health form to await immunization records from your previous school.

3. Send in official transcripts from all institutions attended (including transcripts from the Spring 2005 semester). Once all transcripts have been received an evaluation of your college credits will be processed.

4. Make an appointment with Academic Advisement .

•  Academic Advisement will not be able make an appointment with you unless the above three requirements have been completed.

During the appointment you will work with an advisor to enroll in classes which fulfill degree and major requirements. To prepare for your appointment, please look closely at the semester class offerings at http://www.daemen.edu/offices/registrar/.
