Congratulations on your acceptance to Daemen College! Upon acceptance, you were sent a Student Health Form. If you need another copy of the form, please check below.
Each accepted student must meet certain health requirements, in accordance with New York State law. Any student born on or after January 1, 1957, must submit proof of immunity to measles, mumps, and rubella. Students will not be allowed to register for classes until this documentation is on file in the Health and Insurance Services Office.
Legislation also requires all students, regardless of age, to make an informed decision regarding the meningitis vaccine. Each student must be aware of the risks of the disease, a potentially fatal bacterial infection, as well as the availability of a vaccine to help protect them. Daemen College is required to maintain a record of the following for each student:
- a record of meningococcal meningitis immunization within the past 10 years
* OR *
- an acknowledgment of meningococcal meningitis disease risks and refusal of meningococcal meningitis immunization signed by the student or if a minor, student's parent or guardian.
In accordance with the law, no student will be allowed to attend classes without the required information on file in the Health and Insurance Services Office.
Incoming Student Health Form
Below is the Student Health Form required for health compliance. You must submit this form to the Health and Insurance Services Office in order to register for classes. Download this form if you a freshman or a transfer student.
Visual Effects Program
If you are an incoming student in the Visual Effects Program, you must submit the form below in place of the incoming student health form. This form waives the requirement of a physical exam for admission.

External Learning Experiences
Below are the forms you will need to submit in order to be health compliant for your external learning experience. You must submit the required form to the Health and Insurance Services Office before you attend your first day of the experience. The Director will contact your course teacher regarding student compliance. Please ensure you make copies of your completed form to bring to your clinical site.
