This form is to be completed by visitors intending to be on campus for 30 minutes or more and should be completed BEFORE arriving on campus*.

When you arrive on campus, please check in at the Campus Safety desk (Lobby, Wick Center) or Rosary Hall (campus tours) to complete a short visitor screening form and receive a visitor pass. As of 3/16/22 mask usage is optional. Our mask usage policy, and health screening policy is subject to change.
