Current Open Enrollment for 2023-2024

Our open enrollment period, which begins Monday, April 10th and ends Friday, April 28th 2023 is your annual opportunity to confirm, change, enroll in or cancel your health, dental, vision-care, and life insurance coverage. Except in the case of certain changes in family status or other qualifying events, you will not be permitted to change, cancel or enroll in health, dental, vision-care, and life insurance plans again until the next open enrollment period, one year from now

This is an active enrollment, which means if you wish to have any benefits starting June 1, 2023, you must participate.

Benefit Enrollments that require additional forms to be submitted via the Open Enrollment Portal:

Enroll in the Health Savings Account (Required for NEW Deduct 3 Participants)

If you are NEWLY enrolling in the Deductible 3 Plan this year, you must complete the application process to open an HSA account with Lakeshore Savings, see below:

*Click on the PDF fillable form link, select download or save as (right click your mouse) and save this form to a folder on your computer.  Once the form is saved/downloaded on your computer, close the form.  Open the form you saved on your computer, complete the form and save it again. Once you have completed all of your forms using this method, go to the Benefit Enrollment Portal to upload all of your Open Enrollment Forms and Applications at once.  If you forget to add a form, you can add it later if needed.  If you are submitting a revised form because you made a change, please note this in the comment section on the portal.​

If I choose to have additional HSA funds taken out as payroll deductions (pre-taxed), what do I need to complete?

In order to open an account at Lake Shore Savings for the HSA and comply with the USA Patriot Actemployees will need to provide a copy of their state’s driver’s license or US passport. This is standard practice for all banks.  Employees will need to provide a copy of their spouse’s driver’s license if planning of having a spouse as an authorized user/signer. 

Enroll in the Flexible Spending Account (optional for Copay 1 participants) /Dependent Care Account

If you enrolled in the Copay 1 Plan, and you want to contribute to an FSA Plan, complete an ProFlex FSA enrollment application, see below:
